The Student Health Center staff is pleased to work with you and to assist you in the maintenance of good health. We are available to advise you on how to stay healthy and to assist you when you are ill or injured. Our joint efforts can be maximized if you understand what your rights are. The following brochures provide a summary of the patient rights and responsibilities at the Student Health Center. Please note that you will be required to read and acknowledge the form entitled Patient Consent for Treatment, Acknowledgements and Notice of Patient Rights/Responsibilities prior to your initial visit (see below). This form can be electronically signed on My Student Chart – “Initial Visit Forms” page or when you check in at the time of your appointment.

Your right to privacy includes your right under California state law to have information from an insurance plan related to medical and mental health services that you’ve received delivered to you directly.
